We refer families to health care providers if they do not have established medical homes and to Medi-Cal or Healthy Families if they do not have insurance. The program promotes health and wellness by: Referral to obtain appropriate health insurance Referral to Healthcare Providers Follow up of children with problems identified and assistance with obtaining care from other specialists or agencies Provide resources and quality assurance for CHDP Providers Provides follow up of children in foster care to ensure their health care needs are identified and treated The CHDP program, under the direction of the Children's Medical Services Branch , works with community programs and agencies to identify the major obstacles children in foster care face in gaining access to coordinated, multidimensional services.
CHDP also serves as the local administrator of the state and federal CHDP and Early and Periodic Screening, Diagnosis and Treatment Programs, and works with pediatric medical providers to ensure compliance with child health standards of care. Our goal is to ensure that CHDP provider offices meet state standards in order to provide quality health care for San Bernardino County children and youth. Provider Liaison staff provide training, referrals and resources, in addition to certifying and renewing certifications for CHDP providers.

Care Coordination:. They can help children stay healthily, find problems early, and answer many questions. Through the CHDP program, eligible children and youth receive periodic preventive health exams. Children with suspected problems are then referred for diagnosis and treatment. Many health problems can be prevented or corrected or the severity reduced, by early detection and prompt diagnosis and treatment.
